When agent tries to connect to CTI OS server, it is providing the following failure event:
 
 eCTIOSFailureEvent

Can any of you help to find out the issue. Server has a setting to force login if agent is logged in somewhere else

Try reassociating the phone to the PG user on CM . See if that resolves
the issue. My guess is that your instrument should show out of service
when you execute a procmon on JTAPI .

Even after reassociating the phone, similar issue is occurring randomly when tries to login. For instance, out of 1000 agents - some % are getting this error and goes off after tries couple of times in 5 - 10 mts interval

Server side log also doesnt help much - server side log for the same is attached below

Suggest  you duplicate the issue with the out of box CTIOS Agent desktop and then open a TAC Service Request.

open CMD

type : procmon instance name PG1A or PG1B pim1

type: resetas AgentID

to verify:

type: dagent Agent

you will then see the agent state.
