Kindly see attached screenshot alarm for review. What could be the cause of this problem? What are the troubleshooting tips to isolate further.

It seems there is an critical alarm created for 3 failed attempts under Monitoring and reports > alarms > threshold. You need to look at radius authentication reports to see which user is facing issues with authentication.
