Any idea what might be causing the output to be as below ?

The radius server is alive and half of the machines authorized and other half with below output.

What do the ISE live logs show for that MAC address?  Did you remember to turn User Unknown error condition to Continue for your Internal Endpoint authentication rule?

Authc Failed—The authentication method has failed. This is an intermediate state.

Thus, please engage Cisco TAC and/or Cisco IOS platform support team if the sessions remaining in that state.
