Firmware didn't upgrade across entire stack of C2960s

Was trying to upgrade firmware on a stack of Catalyst WS-C2960X-48FPD-L to the latest version.  I used this link as a guide

Used the copy scp: flash1: command to copy the firmware from my SCP server to the switches.  Ran this command for both flash2 and flash3 to copy across the entire stack.  Ran the boot system switch all command to specify the new firmware version.  Copied run start and ran reload.  But the third switch in the stack came up as not upgrading the firmware.  Ran dir flash3: to make sure the firmware was present, which it is.  On reboot, below is what shows.  The old version is still on switch3 which means it's unusable in the stack.  The first two switches are functioning.  Then tried a boot system switch 3 command to specify the firmware manually on switch3, copied run start, reload.  Still below.

Not sure how to troubleshoot the firmware not showing on one switch.  Any suggestions are appreciated.  Thanks.

Hi,

Does the flash on switch-3 have enough space for the new IOS? Make sure the IOS is actually there.

Also, maybe a good idea to verify the IOS in switch-3
