How to Sign Up For Gold Developer Plan

Does anyone know how I can sign up for the Gold Developer Plan, or have a phone number that I can call?

I submitted a request via the contact form on DevNet but no one has responded.  I sent an email to who I think is our Sales Rep and he hasn't responded.
