You can use extension mobility so that a user has to login to the phone with a username and pin, which will set a different profile on the phone that contains a different calling search space to allow external calls.  The default logged out profile can have a CSS that only allows internal calls.  Alternatively you can use Forced Authorization Codes (FAC) where a user would dial an external number and be prompted for a code to enter in order to be allowed to place the call.
